Medical Care Package - basic information

Dear Members of the JU Academic Community,
please find attached forms of declarations as regards joining the agreement for providing health care services under the agreement for private health care for staff members of the Jagiellonian University (medical care packages), which we send in relation to the fact of signing between the Jagiellonian University and PZU Zdrowie company the abovementioned agreement.
Under the agreement there is the possibility to choose basic package or extended package. Both you and members of your families may benefit from the presented offer (in accordance with regulations of services providing). Please read the offer presenting options of each package, conditions for participation, as well as regulations of services providing. All information can be found on the website of Centre for Personnel Affairs, bookmark ‘Pakiety medyczne’, https://cso.uj.edu.pl/pmedyczne (available for logged-in users).
In order to join the agreement for providing health care services please fill in proper declarations (attached herein and available at the Centre for Personnel Affairs website) and deliver it to the Centre for Personnel Affairs:
1. EMPLOYEE_motion for joining medical care package_EN
2.CHILD under 16 y.o._motion for joining medical care package_EN
3.FAMILY MEMBER over 16 y.o._motion for joining medical care package_EN
If you want to join the contract for the provision of health services from the 1st day of the following month, please provide the CSO (ul. Straszewskiego 27, room no. 5) with the original completed forms by the 20th day of the current month at the latest.
It is as well allowed to send declaration signed with qualified electronic signature to the address: pakiety.medyczne@uj.edu.pl.
